Professional Herbalist Training Program Application

Please note, once the application is submitted, you will need to print it out, sign it, and mail it in with two letters of recommendation and a three hundred and fifty dollar deposit. You may also download the application form to print and fill out by hand. Please include two letters of recommendation and the phone numbers of three to four work related and/or personal references. If necessary, the letters of recommendation may be mailed in separate from the application.
